Welcome to the Elitrasmart Customer Experience Platform (the "Platform"), operated by Elitrasmart Medical Suppliers Limited. By scanning our QR code, accessing, or using this Platform, you agree to be bound by these Terms of Service. If you do not agree with any part of these terms, please do not use the Platform.
By accessing and using this Platform, you accept and agree to be bound by these Terms of Service and our Privacy Policy. These terms form a binding agreement between you and Elitrasmart Medical Suppliers Limited.
The Platform allows customers to scan a QR code at any of our branches, identify the branch they visited, and complete a multi-step feedback wizard rating their experience and the employees who assisted them. Customers are not required to register or create an account to submit feedback. Registration is optional and provides access to the rewards program, activity tracking, and member dashboard.
By submitting feedback through our Platform, you grant Elitrasmart Medical Suppliers Limited the right to use, store, and analyze your feedback for quality improvement, staff training, and business analytics purposes.
We collect and keep the data of the people who take part in the review. This includes your feedback responses and, where you choose to provide them, your name, phone number, email address, and other details. Full details of the data we collect and how we use it are set out in our Privacy Policy.
You are welcome to leave feedback each time you visit one of our branches. A single customer may submit multiple reviews, and you will earn reward points for each completed review. Reward points accumulate across your submissions and contribute to your rewards tier.
Reward points are earned at our discretion and may be modified or discontinued at any time. Points are awarded for completed feedback submissions (including bonuses for rating employees) and are used to determine your rewards tier (Bronze, Silver, Gold, Diamond). Points have no monetary value and cannot be transferred, exchanged for cash, or redeemed unless through an official Elitrasmart rewards program.
Point Expiry: Loyalty points expire 12 months after they are earned. Expired points are automatically deducted from your balance. You can view your point history and any expirations in your activity feed.
Redemption Approval: All reward redemptions are subject to admin review and approval. Points are deducted at the time of request. If a redemption is rejected, the points are refunded to your account. You will be notified by email of the outcome.
Our Platform uses cookies and similar technologies to function correctly and to remember your preferences. Your personal information is handled in accordance with our Privacy Policy. By providing your contact information and accepting these terms, you consent to the use of cookies and to receiving communications from Elitrasmart where you have opted in.

All content, branding, logos, and software on this Platform are the property of Elitrasmart Medical Suppliers Limited or its licensors and are protected by applicable intellectual property laws. You may not copy, reproduce, or distribute our content without prior written permission.
The Platform is provided "as is" without warranties of any kind. To the fullest extent permitted by law, Elitrasmart Medical Suppliers Limited shall not be liable for any indirect, incidental, or consequential damages arising from your use of the Platform.
You may request deletion of your account and personal data at any time by emailing feedback@elitrasmartltd.com with the subject "Request to Delete My Account". Deletion will result in the loss of your points balance and tier status. Please see our Privacy Policy for full details on how deletion requests are handled.
We reserve the right to modify these Terms of Service at any time. We will revise the "Last updated" date above when we do. Continued use of the Platform after any changes constitutes acceptance of the new terms.
